3 PLAYER REGISTRATION FORM

COPY OF BIRTH CERTIFICATE NEEDED FOR NEW REGISTRANTS BEFORE FIRST PRACTICE

I, as a parent or guardian of this child, hereby attest to the accuracy of all information contained on this form and to his/her physical fitness. He/She has my unconditional approval to participate in all VYBA activities during the current season. I assume all risks and hazards incidental to such participation including but no limited to, transportation to and from activities ad I do hereby release, absolve, discharge, protect and agree to hold harmless the VYBA, its director, officers, managers, coaches, umpires, participants, persons transporting my child to and from activities and other individuals involved in the operation or administration of the league from any losses, damages, liabilities and claims arising out of any injury to my child. I will also abide by the code of conduct.

All Coaches and Assistant Coaches must be certified and will undergo a background check. If you are interested in coaching, please fill out the Coach Registration Form.

REGISTRATION FEES

T-BALL: $60

6U & up ( One player + 2 siblings) $225 Total

1 6U and up Player + Two (2) T-Ball Players: $220

2 6U and up Players + One (1) T-Ball Player: $235

3 T-Ball Players: $180

PLAYER 1 SIZES - Please choose all the correct sizes for your first player

PLAYER 2 SIZES - Please choose all the correct sizes for your 2nd player

PLAYER 3 SIZES - Please choose all the correct sizes for your 3rd player

******Please note that coaches will double check your player's size, to what you, the parent/guardian, have provided the league and ask you to confirm.If there are any changes at that time, please do so with our Coach. Once rosters are submitted, there will no longer be an opportunity to change sizes******
